County Agency
The county agency shall maintain a case file of all individuals who apply for the Crossroads Program. The case file shall include the following:
- The Crossroads Program application;
- The notification of decision received from the Department;
- Verification of school attendance;
- Appropriate releases of information.
Department of Human Services
The Department shall maintain a case file of all individuals who apply for the Crossroads Program. The case file shall include the following:
- The Crossroads Program application;
- The notification of decision provided to the county;
- Education Plan;
- Copies of semester evaluations.
